About The Role

The First Contact Team, Brentford and Chiswick, Heart of Hounslow, Great West Road and Feltham Locality team(s) are looking for qualified Social Worker(s) to join a stable and dedicated team of social workers and social work assistants. The teams value the contribution that everyone makes to improve life circumstances of the residents of the London Borough of Hounslow. The First Contact team are the front door team to the service who deal with any unallocated queries. They will either signpost people or refer residents for assessment. The locality teams are long term teams working with a variety of social care assessments to meet resident's needs.

The well-being of the team is paramount creating an environment that is supportive, through offering regular supervision and encouraging reflective conversations. You will be provided with support and guidance from colleagues and managers.

The London Borough of Hounslow has a clear career progression structure. You can have the opportunity to consider applying for the practice Educator training, Best Interest Assessor and Approved Mental Health Practitioner programs. The council also offers a significant amount of additional training to refresh skills and develop other areas of practice.

Our flexible and remote working arrangements allow our workforce to balance work with family and other personal commitments.

About The Team You'll Be Working In

You will be instrumental in carrying out Social Care Assessments providing Care Management support by assisting residents and carers to identify their needs and desired outcomes, helping them to formulate a Care Plan/Support Plan and, where indicated, to use their Personal Budget to set up individually tailored services to achieve desired outcomes.

You will be key in protecting the Human Rights of our residents and carers as much as working with them to promote their safety in line with the Council's and Pan London Safeguarding Policy. You will be instrumental in formulating protection plans, carrying out safeguarding enquiries and participating in outcome meetings.

You will work collaboratively with key stakeholders, care providers, housing, financial services, health, volunteer sector and religious institutions to name a few. There is an expectation that you will have due regard for Equality Diversity of our residents, partners and colleagues.

About You

1. You will have experience of working with vulnerable adults and have competent written and verbal communication skills to be able to deliver timely, person centred and robust assessments, carry out reviews and close cases.

2. You will have strong partnership working skills, working with colleagues, partner agencies, voluntary sector etc. to ensure a holistic assessment is completed.

3. You will be driven to promoting independence, positive risk-taking including person centred Personalised Budgets.

4. You will have competent understanding of and ensure compliance with the Council's and Pan London safeguarding policies and procedures and the Care Act 2014.

5. You will be required to be registered as a Social Worker, and you will work in accordance with values of Social Work England's code of conduct and standards.

6. You will be able to reflect on your practice, participating in decisions where needed to learn from good practice and areas where things could have been done differently.

Qualifications:

• BA in Social Work, DIPSW, CQSW, CSS or equivalent qualification.

• Social Work England Registration
